A devastating public inquiry has found serious failures in management, leadership and safeguarding at the Countess of Chester Hospital during the period when Lucy Letby murdered and harmed babies in its neonatal unit. Lady Justice Thirlwall concluded that earlier action could have saved babies and criticised hospital leaders for failing to act on repeated warnings from doctors. Former NHS Trust chairman Martin Gower says repeated promises that “lessons will be learned” are no longer enough and argues senior figures who fail in their fundamental duty to protect patients should face much tougher accountability.
