More than a million Americans have left the ACA marketplace in 2026 as premium payments spike following the expiration of enhanced subsidies. Data analyzed by KFF show average costs for subsidized enrollees who keep the same plan have more than doubled year-over-year. For many households, that means making difficult trade-offs.
